2-hour response time for new referrals
Average 2-3 RN visits/week/patient
Avereage 4-5 CNA visits/week/patient
Individualized Plan of Care with no limiots on visits for Social Workers, Chaplains, Music Therapy and Massage Therapy
Popen Access Hospice Program
RN caseloads on average 10-12 patients
CNA caseloads on average 7-8 patients
They will get orders, and appropriate paperwork needed to review Hospice eligibility
