Living Well is an evidence-based workshop for people who have one or more chronic conditions. A chronic condition is an on-going health problem or disease such as diabetes, heart disease, arthritis, and other long-term problems. Classes are highly participative, where mutual support and success build participants’ confidence in their ability to manage their health condition to maintain active and fulfilling lives. It is facilitated by two trained leaders in a classroom style, but most of the learning comes from sharing and helping others with similar challenges. It is a 6 week class (2 ½ hours per week) Designed to improve the self-management skills of those living with chronic health issues. People with various health conditions attend together to learn from and provide support to each other. Scheduled at various times of the year throughout Manitowoc and Kewaunee counties.
