Description:
The Ascension Angel of Hope Clinic provides a full range of primary care services. The clinic provides health care for the neighboring residents who have limited access to services, as well as residents at Hope House - a homeless shelter. Services include: routine health assessments, immunizations, well-child check ups/care, screenings for infectious diseases such as TB, HIV, Hepatitis, as well as screening for environmental hazard such as lead poisoning, etc. In addition to medical evaluation and treatment, patient education regarding health maintenance and promotion, sick child care and STD's are key elements of this service.
Patients are seen by appointment only. Medication samples may be provided if available. New patients should bring identification with their home address on it.
