SENIOR SERVICES

- Foster Grandparent Program

Life experience matters.

That’s why SDC is asking adults 55 and older to share their experience with a child. Our Foster Grandparent Program pairs volunteers with young students – pre-school through sixth grade. Volunteers tutor students in math and reading. The relationship that blossoms also helps young students develop self-confidence for life.

- Senior Benefits Program

Age has its benefits.

And programs like Senior Care and Medicare Savings are some of are one of them. At SDC, we enroll seniors that meet age and income requirements in money saving programs including Medicare, FoodShare and Senior Care.

We understand that seniors live on a limited income and that can make it difficult to afford some of the things you need. This includes medication. The Senior Benefits program helps qualifying seniors save money on expenses including premiums for Medicare Part B. The FoodShare program helps to pay for groceries. Keep in mind, many of these programs require that applicants meet Federal Poverty Guidelines.

- Senior Companion Program

The Social Development Commission has a long tradition of working to enrich the lives of senior citizens. In 1974, we started the Senior Companion program because as we age, we may need a little extra help.

Like a good friend, a senior companion provides that extra assistance.

SDC Senior Companions Help Their Elderly Neighbors by Providing:
•Friendship
•Escorts on walks, shopping trips, and medical appointments
•Information on other services
•Help with household services

Requirements:
•At least 55 years of age
•Unemployed or retired
•Interested in helping people

Benefits Seniors Companions receive:
•A tax-free income that does not affect other benefits such as SSI, Medicaid or rent
•Reimbursement for mileage and/or bus fare
•The chance to make new friends
•The satisfaction of helping others

Paid Training Includes:
•An orientation course to give you the tools you need to get the most out of your experience
•Monthly In-Service sessions to help you learn about resources, programs, and health tips for the elderly

NEIGHBORHOOD IMPROVMENT PROJECT

Home maintenance, building code violations or lead safety issues can be expensive. Our Neighborhood Improvement Project program (NIP) can help.

SDC’s Neighborhood Improvement Project (NIP) Program serves income-eligible, homeowner occupied, City of Milwaukee Community Development Block Grant Area families.

In order to maximize the impact of this program, most clients come from lists of households eligible for state and local Energy Assistance program services or City of Milwaukee Weatherization program services.

SDC hires and coordinates building subcontractors who provide moderate home rehabilitations in such areas as electrical or plumbing systems, carpentry, masonry work, etc. Households receiving these home improvements are also offered the full range of other SDC services.

Partial Eligibility Requirements:
•Owned and occupied their property for a minimum of five years.
•Property must be a single or two family unit located in the City of Milwaukee Community Development Block Grant Area.
•Homeowner must be current on property taxes or current with installments.
•Must be current on mortgage and utility payments.
•Must be current on homeowner’s insurance policy.
•Have not filed Chapter 13 bankruptcy in the last six months or have had a Chapter 7 bankruptcy discharged less than a year ago.
•Cannot own more than one property.
•Have not had home repair assistance through any other City-Funded Program.
•Cannot have a daycare or any other home-based business operating within the residence.

- Weatherization Program

Weatherization is a service provided at no cost to the tenant or owner of a home if qualified. Basic weatherization services can:
•Reduce your home heating bills.
•Save energy.
•Make your home more comfortable: warmer in the winter and cooler in the summer.

Some of the services could include:
•Insulation of walls and attics
•Air sealing
•Install energy efficient compact flourescent bulbs
•Test, repair or replace furnaces and water heaters
•Replace inefficient refrigerators with Energy Star units
•Perform a general health and safety inspection of the home
•Provide information about maintenance and energy conservation

How to qualify:

•Applying for Energy Assistance is the first step in determining your eligibility.
•Schedule an appointment with Community Advocates or UMOS (414.270.4653)
•Visit Community Advocates at 4906 W. Fond du Lac Avenue or 728 N. James Lovell Street; or UMOS at 2701 South Chase Avenue
•Your house or apartment (up to 4 units) has not been weatherized before.
•50% of the building’s tenants must qualify for Energy Assistance (Example: A 4-unit building would need two tenants to qualify for the program).
•Your household meets certain priorities that may include a high energy burden or use, an elderly or disabled member or a child under six.
•If you qualify, you will be referred to either SDC’s Weatherization Program, La Casa Esperanza’s or Partners for Community Development’s Weatherization Program.

ADULT & FAMILY SERVICES

Counseling & Wellness Clinic

SDC operates a state certified outpatient treatment program for both AODA (substance abuse) and Mental Health concerns. Counseling services are offered to Milwaukee County youth ages 8-19.

Services Offered
•Education/Awareness Presentations Topics include: AODA (Substance Abuse) Risk Awareness, Anger/Conflict Management & Violence Prevention, Understanding: Mental Health/Mental Illness & Mental Wellness, Dealing with Trauma & Loss, Encouraging Healthy Relationships, Improved Mental/Emotional Health, and Responsibility, through our life choices.
•Comprehensive Screening & Clinical Assessments

Provided by or supervised by licensed clinical staff
•Drug Urine-Screen testing for identified AODA clients
•Outpatient treatment for AODA and/or other mental health concerns
Service provided through individual, group and family supportive counseling, referrals, consultations, case coordination advocacy and ongoing support for youth.
•Celebrating Creativity via Alternative Therapies and Expression

Participants are also encouraged to take action by expressing their creativity through art, music, poetry, a letter, story, theater, mind body awareness-Yoga etc.

Our approach is always strength-based, solution focused, and wellness oriented for our participants. Family participation is encouraged. Our counselors provide education, support, referral assistance and advocacy as needed.

- Substance Abuse

Substance abuse, also referred to as AODA (alcohol and other drug abuse), can be treated through our Counseling and Wellness Clinic. The AODA program is a state certified community substance abuse program which allows SDC to provide treatment to all youth participants.

Program services include:
•AODA education, assessments, counseling and treatment to Milwaukee County youth ages of 8-19.
•Outpatient treatment including individual, and group counseling, case management, referrals, consultations, case coordination, advocacy and support for youth.
•Anger Management counseling for youth in one-on-one or group sessions.

Prevention, Intervention and Education services include:
•Educational presentations within Milwaukee Public Schools, alternative schools, choice/charter schools and other community settings.
•Parent and family AODA education includes presentations, workshops and referrals for parents of at-risk youth.

- Prescription Assistance
. What is the Prescription Advocacy & Referral Service (PARS)

For many people who do not qualify for federal or state health insurance and are unable to pay for their prescription medications, the decision to pay for medication is often secondary to the need to pay for food or housing. Patients who require daily medications often take their medication every other day or cut their pills in half. The existence of prescription medication assistance programs is known to many people, but the complexity of the enrollment process discourages many eligible patients from applying. The Prescription Advocacy & Referral Service (PARS) program was created to guide patients through the application process, and enable them to more easily receive prescription medications.

The Prescription Advocacy & Referral Service is a free service provided to income-eligible clients. Funding for this program is provided by the Helen Bader Foundation and the Social Development Commission. Clients are provided assistance in obtaining free or low-cost prescription medications from participating pharmaceutical companies. The support provided by the PARS program coordinator will include assisting clients with completing and submitting the necessary paperwork and documentation to the pharmaceutical companies with prescription assistance programs that carry the medication(s) they need. Once clients are approved by the pharmaceutical companies, they will receive the medication(s) through their clinic/physician who prescribed the medication. the coordinator will also assist with refills. No medications will be mailed to SDC or kept on the premises at SDC at any time.

PARS will also establish relationships with local clinics and physicians’ offices for the purpose of receiving client referrals for the program and obtaining the necessary client information for processing applications.

The PARS coordinator is also trained to assist with enrollment in health care insurance and FoodShare.

Steps
1.Clients must possess a prescribed medication provided by their doctor or clinic.
2.Clients must call or come in to see the PARS coordinator to make arrangements to start the process of completing the necessary paperwork and providing the necessary documentation requested by the pharmaceutical company.
3.The PARS Program Coordinator will submit the completed paperwork to the pharmaceutical company.
4.After approval, the pharmaceutical company will mail the medication to the client’s doctor or clinic.

Benefits

The main goal of the PARS program is to ensure clients are receiving and taking their medication(s) as directed, so that their health is not compromised. Secondary goals include:
•Monetary savings for enrollees
•Healthy behavior changes
•An increase in health outcomes
