AA Class Licensed community based residential facility assisted living provides residential programming for 8 persons:
HCBS Compliant
Advanced Age
Developmentally disabled
Emotionally Disturbed / Mental Illness
Irreversible Dementia / Alzheimer's
Public Funding
Also provides respite care as needed for people who meet eligibility requirements, based upon bed availability. Accept all public funded clients and private pay.
Services include:
* initial assessment
* annual physicals
* medical appointments
* exercise program and walks
* laundry service
* nail care
* bath care
* semi-private rooms
* meal and snack service
* money management
* medication administration
* transportation
* daily activities
There will be a pre-placement visit with each potential resident to discuss the program and answer any questions. At time of placement, resident will need:
* insurance information
* current medications
* date of last physical and TB test, dentist visit, optometrist visit
* other pertinent information
Residents are reviewed every six months in conjunction with social workers, guardians and concerned family members.
