Information about Handicap Parking Cards is available at the local DMV office during office hours, via the website, or by phone via the Statewide Information Center 24 hrs./day. People may also call for information about licensing for drivers with special medical or physical conditions. To apply for a Handicap Parking Card:
* Get an application form from the DMV and have a physician fill it out
* Return it to the local DMV office with the appropriate payment
* No fee is required for issuance, renewal or replacement of a DIS ID permit issued for a permanent disability. However, applications made at a local DMV service center that provides DIS ID permit service are subject to a counter service fee.
Eligibilities: Application signed by medical personnel.
