Advance registration for body donation is required for consideration of acceptance at time of death. Donor needs Plan A (UW) and Plan B (Other disposition if refused) IF a person's physical condition is not usable for education here. Call or write for more specifics/explanation. We require use of our UW form, which will be sent with explanatory handout, upon request of interested parties. Or give us your phone number so we can call you back with information and answer any questions you may have.

We prefer not to provide our handouts to third parties, because we update forms and handouts periodically, and prefer not to receive old outdated forms from prospective donors. Interested persons should call or write for current information and handouts. (608) 262-2888. If leaving a message, leave your name (spelled) and address on the answering machine. We respond within one to two days of a request. Phone number is not needed when calling or writing unless you have a specific question you need answered prior to consideration for registering. Requesting our information does not obligate a person to register as a donor.
