Here's what you can do
To get your original (first) Wisconsin ID Card, you must visit a DMV Customer Service Center.

The following will be required:
__ Proof of legal status in the U.S.
__ Proof of Name and Date of birth
__ Proof of Wisconsin residency
__ Proof of Identity
__ Proof of name change(s) (if applicable)
__ Provide your social security number (if applicable)
__ Complete an application for an Wisconsin Identification Card - form MV3004
__ Fee (if applicable)
Note: If you have held a Wisconsin driver license in the past eight years and now wish to apply for an original ID Card, you may be eligible to apply online or at a DMV service center ( proof of identity is required).

ID Petition Process:
If you are applying for an original (first time) Wisconsin Identification (ID) Card for voting purposes and your documents to prove U.S. citizenship, name and date of birth, or legal name change are unavailable to you, you may use this process to apply for a free ID.

The Document Verification Petition Process is available if you are unable to provide the following:
Documents for proof of name and date of birth
Documents for proof of United States citizenship
For applicants using this process, you must visit a DMV Customer Service Center The following will be required:

__ Proof of identity
__ Proof of Wisconsin residency
__ Complete an application for an Wisconsin Identification Card - form MV3004
__ Complete a Document Verification Process (DMV Administrator Petition) form MV3012
Once you start the application process, the information provided on form MV3012 will be used by the DMV to communicate with state and federal partners in attempt to verify unavailable documentation.

Wisconsin DMV issues REAL ID compliant products (marked with a µ) in accordance with the federal Real ID Act of 2005. If you plan to fly within the U.S., visit a military base or other federal buildings, the Department of Homeland Security will require identification that is REAL ID compliant (or show another acceptable form of identification, such as a passport) beginning October 1, 2021, the new deadline for this Federal requirement.
